Debt Submittal Form: Complete this form when submitting accounts to us for recovery. Use of this form is not required if the listed information can be provided to us in some other clear and concise format. We encourage our clients to contact us to see if their documentation will provide us with the information we need in order to enter an account. You must already have a Collection Agreement on file with us in order to forward accounts to us for recovery.
Debt Submittal Form (when monthly charges continue to accrue): Use this form when submitting accounts to us for recovery if the written contract between the client and the debtor specifically permits the addition of monthly charges. This is common for accounts originating out of a contract for a residential or commercial lease, HOA fees, membership dues, etc. Accruing monthly charges must be specifically permitted in the written contract agreed to by the debtor and the client and a copy of the contract must be provided to us. You must already have a Collection Agreement on file with us in order to forward accounts to us for recovery.
